FLU SHOT INFORMATION - 2011-2012

Flu Shots for WCSD Students and their families:

 

WCSD will be hosting after school hours Seasonal Flu Shot Clinics for students (children 5 years old and older) and their families.  All children must be accompanied by their parent/guardian.  This clinic is offered by Workplace Vitality Flu Prevention Partners.  This is not a free clinic, insurance is required.  For more insurance information:

There is NO SIGN-UP.  This is like a community clinic, first come, first served for the convenience of our students and families. 

 

Price:  Insurance card covers the cost for the most major insurances (Including Blue Pt. 2). YOU MUST BRING YOUR INSURANCE CARD WITH YOU.  Without insurance card the charge is $30.00 cash per person. Checks can be made out to “Workplace Vitality”.  NO CREDIT CARDS ARE ACCEPTED!!!

 

You and your child may receive all immunizations (no cost for students) at: Monroe County Health Department, 111 Westfall Road, Rochester, NY  14692 Immunization Clinic (phone:  753-5150). Walk-in clinics are Wednesday afternoons 12:00-7:00 pm with the last check at 6:30.  Students should have their immunization record with them. Students enrolled in Child-Health Plus, or Medicaid must bring their insurance card.
